- Your personal contact details including real name,
stage name, address and email addresses.
- Details of your current and your most recent residencies - Name of venue, town/city, date started, and date finished.
- MINIMUM of two personal references that have given you their permission for us to contact them – In both cases YOU MUST SUPPLY: Full name, a valid email address, contact telephone number, employer and position (minimum status - venue manager).
- PHOTOGRAPH: JPEG format min 170 pixels wide by 170 pixels high. Note: Your photograph will be published on the website, therefore, only a good quality HEADSHOT is acceptable. If cropping and resizing your photograph is a problem for you then email your photograph to us direct on mail@prodjs.co.uk.
- Your desired fee for - Fri/ Sat 2am residency, Sun/Thu residency, 3 hour set (single booking), Full mobile gig (if applicable).
